There is a common myth about this, stemming from how creatine works: by increasing its muscle use for energy, it generates a waste product called creatinine. This creatinine is released into the blood and filtered by the kidneys in a completely physiological and natural way. If you have a blood test, creatinine may appear elevated simply because your muscles are producing more waste, not because the kidneys are failing.

The myth stems from a single isolated study in 2009, conducted on only 20 people for 21 days. It used a very high loading protocol (20 g/day) and observed an increase in dihydrotestosterone (DHT), a hormone linked to hair loss. However, that study had significant limitations: hair loss was never measured, only hormone levels. Furthermore, the increase in DHT remained within normal limits, and its results have never been replicated in subsequent trials.

But most importantly, in April 2025, the first controlled clinical trial evaluating this directly was published.* (Lak M, et al. J Int Soc Sports Nutr. 2025). For 3 months, the use of 5 g/day of creatine monohydrate was compared to a placebo in healthy young men. Hormonal and renal biomarkers (testosterone, DHT, creatinine) were analyzed, in addition to direct hair health through dermatological trichoscopy.

Results: There were no significant differences between creatine and placebo in DHT or testosterone hormones, nor in hair loss. No adverse effects or alterations in normal kidney function were observed either. Thus, this study directly rules out the idea that creatine causes hair loss, debunking the old hypothesis from 2009.
